Medication reconciliation is a critical yet often overlooked process in patient care. It involves comparing a patient’s current medication list against the medications they should be taking, helping to prevent errors and ensure safety. I recently attended a workshop that provided some excellent strategies for effective communication during this process; I’d love to hear what practices others have found beneficial.
